Dear Patient,

You have received a text message with a link to this information because you are on one or more of our chronic disease registers. A chronic disease is also referred to as a ‘long term condition.’ This means your condition requires you to have monitoring and treatment for a long period of time, often for the rest of your life.

Conditions such as:

  • Diabetes
  • Pre-diabetes (non-diabetic hypoglycemia)
  • Coronary Heart Disease
  • Hypertension (high blood pressure)
  • Respiratory conditions such as COPD and Asthma
  • Rheumatoid Arthritis
  • Mental health conditions
  • History of having a stroke

Many patients have one or more chronic disease along with other requirements such as medication monitoring. This often resulted in patients receiving a number of invitations to attend a number of different appointments. We aim to book as few appointments for you as necessary to manages your long term conditions.

Frequently asked questions

I have recently attended for a review, why are you asking me to attend again?

Every 1st April 2021 our yearly review calendar resets. From April we will invite you to attend within your birth month for your long term condition review. This review will aim to make sure we have reviewed all aspects of your health at one appointment where possible.

Why do I need to attend?

Even if you are feeling well it’s extremely important to make sure that your condition is well controlled and that you are taking the correct, most up to date medication to help control your condition.

What if I don’t want to attend?

We strongly recommend that you do attend. If you are on long term medication, your GP may not feel it safe to continue issuing this without you attending for a review of your condition.

What do I need to bring to my review?

Some reviews, such as those for respiratory conditions, require a nurse to check that you are taking your inhaled medication correctly. You will need to bring your inhalers/devices with you to your appointment if you are being seen face to face.

Our aim is to achieve the following –

  • Bring medication reviews and annual chronic disease reviews in line with one another to simplify and improve communication and processes between patients, clinical staff and the teams that dispense medication
  • Reduce duplication of attendance to the Health Centre for patients
